The initial appointment at the Oral & Facial Surgery Center of Puyallup will consist of a consultation explaining diagnosis and treatment options. Occasionally, surgery can be performed the same day as consultation. In most cases surgery is performed after evaluation of medical history and treatment plan.
A Consultation Will Require The Following:
- A surgical referral slip and any applicable x-rays
- A list of current medications being taken
- Completed medical or dental insurance forms. This saves time and helps to process any claims.
All patients under the age of 18 must be accompanied by a parent or guardian during the consultation visit.
A consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please do not eat or drink eight hours prior to surgery. Also please ensure you have an adult to drive you home after the surgery.
Please alert the office prior to surgery if you have medical conditions of concern (i.e. high blood pressure, diabetes, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, etc.).
If your current dentist or physician has taken x-rays, they may need to be forwarded to our office or brought in by hand.
